I checked your site http://www.auxiliumart.com/ but didn't get any pop up there.
The WordPress PopUp plugin doesn't provide any contact form so could you please tell me how you have implemented it into the pop up? using any plugin or developing custom code?
It was working great until today. What could it be?
Have you added/updated/deleted anything recently that caused this issue?
