I think the plugin Contact Form 7 can help here. See this link:

You would just create a new contact form for each artist, generate short code for the post, then copy/paste the code into post and/or page.
Another popular Contact Form plugin is Gravity Forms (http://www.gravityforms.com). I looked around their Support but couldn't find anything specific. But I am pretty sure it can spin off multiple contact forms and use one per artist just like advised above.
