suzyloonam on "[Plugin: Contact Form to Email] Form doesn't show in desktop Chrome"

Hi, Using Divi theme on WP 4.2.2, the form does not show in the latest version of Chrome on my desktop. Works okay on Chrome mobile. Form does not show when I change to Twenty Fifteen, either. Java Script is enabled. Flash is enabled. Cookies are enabled. Is there a fix? http://riverbrite.com
