Working on a survey design, but have popping up next to check-box and text of 'option' ... not sure how to get rid of all these displays of in the survey when viewing it.

I typed in the [] to show that the checkboxes are appearing, but I cannot figure out why there is a displaying - it is not coded in like that, and doesn't appear anywhere. HELP PLEASE. Page on website is currently private as the form is not launched.
