no idea what you are talking about. this is a contact form widget and if you want to display the widget inside a page you should use https://wordpress.org/plugins/siteorigin-panels/ - by the same developers which allows you to insert widgets into pages.
If that's not what you are looking for, please explain again what you're looking ot do, ideally in your own thread.
